The purpose of the Aims2UNC Student Participation Agreement (“Agreement”) is to communicate the academic and advising expectations, as well as the scholarship processes and requirements of the Aims2UNC Program (“Program”), and outline the policies and procedures that will apply to participants in the Program
1. Completion of this Agreement is required for full admission to the Program.

Additionally, signing this agreement serves as acknowledgement you understand failure to meet any of the expectations outlined below may result in removal from the program.
I acknowledge it is my intention to complete an associate’s degree (Associate of Arts, Associate of Science, or approved Associates of Applied Science) at Aims Community College (“Aims”) and transition to the University of Northern Colorado (“UNC”) to complete a bachelor’s degree.
I understand if my intentions or goals change, and I no longer plan to earn an associates degree from Aims or a bachelor’s from UNC, it is my responsibility to let an Aims2UNC staff member know.
Academic Expectations
While at Aims, I understand that to maintain good academic standing, I must achieve a cumulative 2.0 GPA or higher. If my cumulative GPA falls below a 2.0, I understand additional requirements may be placed upon me, centered on improving academic performance, beyond what the institution requires, in order to stay in the Program (including but not limited to, tutoring, study skill development, workshops, peer coaching, and additional support services). If my cumulative GPA continues to be below a 2.0, and I am placed on multi-term probation, I understand that I could be dismissed from the Program.
I understand that if I do not enroll in classes at Aims for three consecutive semesters (a full academic year) I will be dismissed from the Program. If dismissed, I will be required to reapply for the Program.
Advising Expectations
In order to assist my Aims2UNC Advisor as they help me achieve my educational goals, I agree to notify Aims2UNC staff of any changes in my contact information and follow the additional steps to update my student enrollment records at Aims. I agree to check my Aims and UNC email accounts regularly (as I understand that such email accounts are the official communication methods of Program participants), check the Aims2UNC D2L course shell regularly (participants will be added to automatically upon acceptance to the Program and can be found on my My Aims page), respond promptly to communications from Aims2UNC staff members, and complete the Program survey at the end of each academic year.
I agree to meet with my Aims2UNC Advisor a minimum of twice a semester to develop, review and update my academic progress and transition plans, and to discuss additional support services (including but not limited to peer coaching, workshops, financial aid, career counseling, and personal counseling) that may prove beneficial to my academic success.
I understand it is my responsibility to schedule an appointment to meet with my Aims2UNC Advisor, to come prepared, and to participate actively in the meetings with my Advisor.
Scholarship Requirements
I understand the Aims2UNC scholarship is an institutional scholarship while I am a degree-seeking student at Aims. I understand that to be eligible for the Aims2UNC scholarship at Aims I must complete all of the following by the last day of the current semester:

maintain a cumulative GPA of 3.0 or higher,

attend a minimum of two advising appointments with my Aims2UNC Advisor,
-
participate in a minimum of two Aims2UNC engagement activities,

complete a minimum of twelve (12) credit hours at Aims, and
-
register for a minimum of six (6) credit hours for the following term. The Aims2UNC scholarship amount is based on fulltime enrollment (12 credit hours) and the amount will be recalculated for those enrolled in at least 6 credit hours.
I understand UNC has a separate scholarship application and the UNC scholarship requirements are different than the requirements listed above. An Aims2UNC staff member can help me review UNC scholarship requirements near the point of transition, but I understand that I am responsible for completing the UNC Scholarship Application.
Use of Name, Likeness and Recordings
I authorize, in perpetuity, Aims and UNC to use my name, photograph, video and/or audio recording of me in any Aims or UNC written, video and/or audio materials of any type or manner for educational or promotional purposes of either Aims and/or UNC.
UNC Student Code of Conduct
As an Aims2UNC student I understand I must conduct myself in accordance with both the
Aims Student Code of Conduct and the
UNC Student Code of Conduct (BEAR Code). Under the BEAR Code, a person is a student from the time they are issued a Bear Number until the time they are awarded a degree by UNC or until their date of withdrawal from UNC, whichever occurs last. Until a degree is awarded or a person withdraws from UNC, a person’s student status continues uninterrupted. It is important to note that the BEAR Code applies to misconduct by a student that occurs on UNC property or off-campus and includes misconduct that occurs between semesters or when classes are not in session.

Immunization Records
I understand as a non-degree seeking student at UNC, I must submit proof of required immunizations to the UNC’s Immunizations Office (email: debra.miller@unco.edu, fax: 970.351.3234 or 970.351.2427; or mail to UNC Immunizations Office, 501 20th St., Campus Box 46, Greeley, CO 80639).

3. If you cannot locate immunization records you must contact the Immunizations Office at UNC to discuss other options. If you choose to not submit proof of immunization, you will need to fill out a medical or non-medical exemption form in order to participate in the program

2Aims2UNC Participants born after January 1, 1957 must show proof of two MMR (measels, mumps, and rubella vaccinations, the first administered no earlier than 4 days prior to the first birthday, and the second at least 28 calendar days after the first.

4The Medical or Non-Medical Immunization Exemption form can be downloaded at: www.colorado.gov/pacific/cdphe/vaccine-exemptions. If a Program Participant submits an immunization exemption form and there is an outbreak of a disease on the UNC campus for which the Participant has received exemption form immunization, such Participant may be subject to exclusion form the campus.
Official Transcripts
I understand UNC will require official transcripts from all previously attended universities and colleges prior to my transition to UNC as a degree-seeking student. I understand if I submit all official transcripts from all previously attended universities to Aims, a copy of those documents will be shared with UNC as a part of my Aims student record at the time of my transition. I understand if I do not submit official transcripts from previously attended colleges or universities to Aims prior to my transition, I will need to independently request and submit those transcripts to UNC’s Admissions Department. Failure to submit official transcripts prior to my transition will affect my ability to become a degree-seeking student at UNC

5A transcript is considered official if it is sent directly to either Aims or UNC from a previous institution (electronically or via postal mail), or, if it is delivered in a sealed envelope.
UNC Academic Requirements
I understand that maintaining a cumulative 2.0 GPA or higher, and earning a minimum of a C- in each class that will be applied to a UNC degree will position me for a seamless transition. However, I understand that some UNC the Programs of study have additional or different admission requirements (please see:
www.unco.edu/admissions/how-to-apply/transfer), and that in order to be admitted to any program at UNC I will need to meet all program specific requirements.
Transportation Provided by Aims
I understand that Aims may provide transportation for me to and from the UNC campus for certain activities regarding the Program and in consideration of Aims providing such transportation, I release and discharge Aims, UNC, and their respective Trustees, officers, employees and agents from all claims, demands, causes of action and damages whatsoever, relating to any injury or other harm or loss, to person and/or property, arising from the provision of transportation by Aims.
